What is the Statue of limitation for receiving permanent Disability benefits for workermen comp with the County School System.

I was hurt by students running to be the first on the bus on 7-11-05. I have had at least 11 surgeries and contracted an infection in the hospital. I have had six surgeries on my hip four on my knees and one on my right shoulder . I now have two torn shoulders my right hip is also in need of a hip replacement and I am presently doing electro therapy because the company refuses to do anymore surgeries. I am retired because of the pain and receiving a disability check and considered permanently disabled but have not received paperwork stating that I am permanently disabled. I have asked for it to be provided but I am ignored or told they will get it but I have never received that information.
