There is no form. You need to draft a SUIT, also known as a COMPLAINT, which must comply with the FEDERAL RULES. It is not easy, and you run a risk of having your suit dismissed on a technicality. I strongly urge you to get an attorney. Most discrimination attorneys do not charge just to talk, and, if you have a good case, will not charge unless you win. They may be able to get their fees paid by the sleaze-ball employer.

This is not a matter of filling out forms. You must actually draft a complaint from scratch and comply with all of the court's rules. This is really something you need a lawyer to assist you with. Proving retaliation is much more difficult than many laypeople believe.

You must first file a new charge of retaliation with the EEOC. If your claim has merit, you should be able to find an attorney to take it on.
